Cultural approach to illness and pain - conception differences between the occidental model and the Melanesian and Aboriginal traditional model



Recently, I have been participating to a lecture about indigenous people. The lecturer described aboriginal culture and their language of pain and barriers with health care professionals but all along the lecture I felt there were a real connection between Aboriginal and Melanesian people. There were multitude similarities regarding their culture and behaviors. There were also similar stereotypes from Health care professionals and the community.

I then realized that Melanesian people may face similar barriers with health professional in New Caledonia. Unfortunately not much research about this issue has been done and I am not even sure if anything has been considered to assist them. However, this would be essential to facilitate treatment managements and processes.

I am currently writing an article listing similarities found between aboriginal and Melanesian cultures and beliefs. My goal would be to examine these similarities to find out if the Melanesian community face same obstacles and share same feelings regarding pain management as aboriginal people and if anything can be done to remove these barriers.

From the research I have done at the moment, I found that they face similar language barriers (no equivalent words in English or French in their own language, and they have a word that covers everything from pain to any type of injury and illness with the differentiation made by the context in which the word is use, "pika" for western Australia aboriginal communities and "aketr" in drehu language for Lifou Melanesian communities).

Melanesian people also have similar beliefs towards illness and pain as aboriginal people. Illness is physical, social and mystical. They believe there are 3 different type of illness: natural, caused by spirit ancestors, caused by a "boucan" (evil spell from a jealous person through a sorcerer) and to be treated they need their own traditional medicine and family.

Both do not verbally complain about pain because they do not want to be seen as weak, or be ashamed of saying the wrong things. They often hide their symptoms from others, delaying diagnosis or not pursuing treatment because they can feel ashamed of "wrong doing" (Melanesian and aboriginal people have this notion of being sung, it's basically a bad magic put on somebody because of breaking a "taboo" or law).

Both communities suffer from stereotypes (i.e. "they don't feel pain", "they don't understand or get what I am trying to explain", "they don't care of pursuing treatments", "they cannot explain in details") and communication barriers with health care professional: they speak a different language, have different time concept, respect concept, silence time, body language (averting gaze, shrugging eyebrows for example are forms of respect and humility in the Melanesian society). They are shy because their culture reinforce humility and respect especially with older people. Silence and listening are parts of their entire childhood, during this time they cannot say whatever they want to say to anyone they want, it is inappropriate to express their opinions to people older than them.

These concepts and cultural differences are often not all well understood and some behaviors can sometimes be seen as "rude" for the health professional. But aboriginal and Melanesian people may also feel that HP are not interested about their beliefs, tragic past events, traditional medicine and culture and both can feel HP are not listening to them, do not believe in them or do not care.

We need to develop better communication techniques that build trust and consideration,we need to be culturally sensitive and respectful for better treatment management and process and equality towards all communities.

New kid on the block

Hi, my names Can (pronounced as 'Jarn') Craddock, and I'm 22 years old. I'm currently studying a Bachelor of Clinical Exercise Physiology at Queensland University of Technology and enjoying it a lot. I played a bit of Opens Boys Union and Touch in High School and competed in Gateway Cross Country and Swimming. I also did some recreational boxing at 'Shocking Boxing' during my senior years. Since leaving High School and entering University, I have competed in a couple of fun runs and completed my first marathon last year. I have family and friends overseas and plan to travel through Europe in the summer of next year. I'm excited to spend my prac hours at Fitnance and look forward to meeting all the staff and regular clients.

Speed bumps for the Galloway's


Starting with the flight to Hong Kong, things weren't going to plan for father and son, Fraser and Angus Galloway and their entry for the Regetta. Starting with no paperwork to enter European air space, the boat owner also did not supply rudder box and tiller.

Race day 1 - Things continued to go wrong when broken mast step left the guys towed home.

Race day 2 - After some communication errors, the guys finished 6th and placing 11th overall.

Race day 3 - With continued no luck, the guys ended up capsizing and turned around.

With three races left, Fraser comments how this is the worst and most expensive Regetta in his life.

Fraser concludes that tomorrow they will venture back into the valley of death with the intension of taking no prisoners.

Patchy's Tip of the Week

Hello my name is Nelson Patchett, I am currently a Trainee at Fitnance and welcome to the first installment of Patchy's Tip of the week.

This is dedicated to the up and coming body builders out there who are keen to increase gains in body size and strength.

When performing high intensity hypertrophy training, it helps to use a technique called the Anti-Lock. This is simply not straightening your joints to maximum extension. For example, on the Bench Press, this will increase the total time under tension with maximizes the promotion of muscle fibre tear as well as limit any unintentional rest time that is created when your joints lock out. This is able to be done with all your bar bell and dumbbell exercises such as seated shoulder press, dumbbell bench press and squats. Though this will hinder your ability to lift heavy weights, it will ensure your muscles are being torn to shreds on a microscopic level. Therefor promote muscle growth in short term. Drink lots of water, eat lots of healthy food and get lots of rest.

Try this during your next workout, feel the burn and have fun!

Cardiovascular disease and diabetes in ‘skinny fat’ people

Ever found yourself wondering how unfair it is that some very thin people can eat all the bad food that they want and it never seems to be stored as fat anywhere on their body? Well the fact that we notice we are gaining more weight may actually be a blessing in disguise (provided we do something about it). Researchers have found that just because a person is thin, doesn’t mean they aren’t fat. Thin people that eat junk food without exercising, yet never appear to never gain a kilo are most likely storing fat around their organs and in their arteries. MRI scans have further backed up this theory, showing sedentary people of normal weight possessing much larger fat deposits around their organs than those classified as overweight, regularly active people. In fact, in this study performed they found that out of the women scanned, 45% of them with normal BMI scores had excessive internal fat deposits, and 60% of males within the normal BMI category had excessive internal fat deposits. As this is not visible to the naked eye, these people find it harder to notice how their poor diet and lifestyle is affecting their body and as they have no visible indicator they cannot see how much harm they are actually doing, therefore do not see a reason to change their habits. The reason these thin people do not store their fat as visibly is not because they have a faster metabolism. Usually it’s because they eat too many sugary fatty foods without performing any physical activity, but aren’t eating enough to actually be classified as fat (on the BMI scale or to the naked eye). Links have been established between greater internal fat stores and the risk of cardiovascular disease and type 2 diabetes. In addition, fat stored around the organs is thought to disrupt the body’s communication systems, disrupting vital signals to organs such as the pancreas and liver. This may alter insulin secretion, thereby increasing the risk of diabetes. This risk of heart disease is also elevated. Excess fat in the arteries (associated with increased internal fat deposits) are associated with increases in blood pressure due to increased resistance to blood flow through the arteries and the risk of developing atherosclerosis (hardening of the arteries) is increased greatly. This increases the risk of hypertension and heart disease. Through improvements in diet and participation in regular exercise and physical activity, these levels of internal fat storage can be markedly decreased. In fact this is one of the first adaptations to increasing physical activity levels. Whilst there may not be obvious reductions in body fat initially, mobilisation and oxidation (transport and use of fat as energy) is greatly increased with increased physical activity levels. By maintaining this healthy lifestyle, risk of cardiovascular disease and diabetes is greatly reduced. Above all else, the most important thing is to stay active.
